Important Forms and Instructions
Our goal is to make getting quality medical attention as convenient as possible for patients. To help you prepare for your first appointment, review the following details carefully. We look forward to assisting you with your cardiovascular care.
First Appointment
Allow 1-2 hours for your initial visit. We ask new patients to arrive 15 minutes early to finalize all forms. Please bring your insurance cards and photo ID.

Patient Information
To provide you with optimal care, please complete your patient registration and medical history forms upon arrival. You may complete these forms, or you may download, print, and complete the paper form and return it at the time of your appointment. We also ask that you bring all of your medications in their original labeled bottles.
Patient Testing Instructions
Cardiovascular Medicine offers two noninvasive Diagnostic Testing Centers. The facilities are located on the 2nd floor of the Heart Institute in Davenport, Iowa and on the first floor of the Moline, Illinois location. Medical Records
Release of information from your medical record must meet legal requirements and Cardiovascular Medicine policy.
Financial Information
Co-payments and patient balances are due at the time of service. For your convenience, we accept personal checks, Discover, MasterCard, and Visa. We also participate in many insurance plans, including Medicare.
